Vulva Aching When Aroused

Hi Everyone,

I've noticed this weird thing lately.  It's actually been happening for the past several months.

First, some background. I am 25 years old and I've never been pregnant, and I was on birth control pills for over seven years, but I got a Paragard IUD four months ago.  I've had this problem for several months though, so it started before I got off the Pill.  I actually switched birth control methods because of a low libido (that I've struggled with on pretty much every single birth control pill that I've ever been on, but I just dealt with until now), and while my sex drive has definitely come back a significant amount, I don't think that I'm back to 100% normal yet.

Anyway, now to my problem.  When my fiancé and I first start making out, at the point where it starts getting intense and I really start getting turned on, I experience an aching type of pain down below - in my vulva, and probably mostly centered around the clitoral area.   I can tell that I'm turned on, but it's somewhat painful.  Not unbearable, but not particularly pleasant.  I imagine that it feels a lot like what 'blue balls' must feel like.  It's just really weird, because it's only been happening for the past several months, and it does not happen when I'm alone, fantasizing or whatever.

The pain usually resolves after a few minutes, as we continue making out.  It definitely never hurts once he starts touching me sexually - so it's always right at the beginning, when things start heating up.

Does anyone have any ideas about why this is happening?  Am I not having orgasms frequently enough, and it's causing a 'blue balls' equivalent type of thing?  Ever since I've been on the Pill (so for many years), I have often gone a week or two without masturbating, but never had this problem before, so I don't understand what's going on now.  I also never used to experience this problem with my fiancé, nor have I ever experienced it with any other guy.

For background:  my fiancé and I have been together for three years, and we're in a completely monogamous relationship.  We have both tested negative for HIV, herpes 1 and 2, chlamydia, and gonorrhea.  My last pap came back normal, and I also tested negative for the strains of HPV that a lab would typically test for.  My last exam (although not a pap) was less than a months go, and everything looked and felt normal to the practitioner.  I have had BV and and a yeast infection over the past several months, but those have been resolved (confirmed by a swab done during my IUD check-up).
